Step 1: Identify individual/Family in crisis Step 2: Complete the online voucher using the below link: - Partner link: https://forms.gle/imD9YXTqRb23jBVK6 - Self-referral link: https://forms.gle/DoJYEL2RNXHwz2bT7 Step 3: Once the voucher is received we will reach out to the individual/family confirming what day they will be attending. Step 4: We will provide the food. A weeks worth of supply. Step 5: We will work with the individual/family to get them through the crisis that they are going through Restrictions: Decided by each case independently.
